Changing your contact details

To update details like your name, address, phone numbers or email address:

• Use our Online Access Admin Form
• phone or visit the surgery from 8am to 6.30pm

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

• Check you are in our catchment area

Please do remember to keep us informed if you change your phone number or email address. You can let us know this by completing an Online Access Admin Form

You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate. Please bring these documents in to the Practice and speak to the Reception team who will be able to assist. We will need to see original documents.

In rare circumstances when a date of birth error is identified, we will need official proof  to rectify any mistakes with the National NHS teams. A passport is a suitable document for this.
